Role Purpose

As a Housing Solutions Officer (Hospital Discharge), you will work proactively and collaboratively with individuals who are homeless or at risk of homelessness upon discharge from hospital. Your role will be to provide comprehensive housing advice, prevent homelessness where possible, and help secure alternative accommodation when needed.

You will work with a diverse range of households, including private and social tenants, homeowners, and those experiencing family or relationship breakdowns, many of whom may have complex needs or vulnerabilities.

Key Responsibilities

Service Delivery

  • Provide high-quality advice and support to prevent homelessness wherever possible, in line with relevant legislation, statutory guidance, and Council policies.
  • Manage a caseload of applicants under the Homelessness Reduction Act 2017, ensuring timely and effective actions to minimise temporary accommodation stays.
  • Make well-reasoned decisions regarding homelessness applications and facilitate appropriate referrals for long-term housing solutions.
  • Collaborate with local hospitals to ensure seamless hospital discharge planning and effective homelessness prevention interventions.
  • Arrange temporary accommodation for eligible homeless applicants and ensure compliance with property standards and rent account management.

Partnerships & Collaboration

  • Build strong working relationships with key stakeholders, including:
    • Local hospitals and healthcare providers
    • Private landlords, housing associations, and social landlords
    • Social Services, Police, Probation Services, and voluntary organisations
  • Attend multi-agency meetings and ensure timely updates on case progress and follow-up actions.
  • Identify and raise safeguarding alerts, ensuring appropriate referrals to support services related to income maximisation, debt management, and employment opportunities.

Performance & Innovation

  • Support continuous improvement by identifying best practices and innovative solutions in housing and homelessness prevention.
  • Monitor and report on performance targets, trends, and case outcomes, contributing to service planning and development.
  • Stay up to date with housing-related legislation, case law, and policies, offering specialist advice to colleagues and partners.
